Add displ tag to stack pointer offset annotations to avoid ambiguity in case the same constant offset appears twice in an instruction.

Add SMPFunction::FindAllocPoint method to find end of stack frame allocation when it is done by pushes, e.g. in strpbrk().

Emit annotations for orphan code bytes that were still isUnknown after all other fixups; minor change to ad hoc debugging address.

Provide SMPInstr methods to access uses and defs externally; add methods EmitAnnotations and MDUsesCalleeSavedReg.

Redefine stack regions from IDA perspective to ours by getting all callee-saved regs out of the locals area; move emission of annotations to SMPInstr class.
